Pre-Camp Paperwork

Please review the following pre-camp forms and sign as needed. These forms must be submitted at least three weeks before your camper arrives at Hawthorne Valley Farm Camp. Once your forms are complete, please upload a copy to your parent portal or email them to farmcamp@hawthornevalley.org.

• Physician’s Health Form - All campers must have a physical exam in order to attend camp. All health exams must be dated within 364 days of the last day of your child’s camp session.

• Letter for Meningococcal Meningitis - The parents/guardians of campers entering 7th grade and up in Fall 2024 and attending camp for 7 or more days must read and sign this informational letter about the Meningococcal Meningitis virus.

Please note that if we do not receive these forms prior to the first day of camp, according to NY State Law, your child will not be admitted.
